Application for Referral Letter

A referral letter is sometimes required when visiting the libraries of other universities and institutions. The campus library will inquire in advance and issue a referral letter if required.
Pick up your referral letter at the counter after you receive e-mail notification that the letter has been issued. (Referral letters cannot be mailed or sent via campus mail.)
Eligibility for service: Enrolled students (excluding working auditing students and certified nurse trainees), faculty and staff

Note:
  1. It will take approximately one week to issue the letter. Be sure to submit your application well in advance.
  2. You may request up to three documents using this form. If you would like four or more, please submit the form again.
  3. Contact the library to cancel requests or if you have not heard back within a week.

Note: If there are multiple libraries at the university you wish to visit, provide the name of the specific library as well.
